If you live within the practice area you may apply to register at the practice. Registration forms are available at either Wadebridge or Rock surgery.

If you change your address or name please notify the surgery as soon as possible. This will enable us to contact you if the need arises.

We also find it very useful to have your telephone number, including mobile telephones, and postcode.

Please note that if you are having any hospital treatment you will need to inform the hospital of your change of address or name.
