New Patient Registration
Patients living within the practice area are very welcome to register with us. Please use the registration forms available to download (below) from this site. Signed registration forms should be brought to reception. Visitors and holiday makers may register for temporary services only. Please note that these are limited to the duration of your stay and that you should see your own GP on your return home.
When registering as a new patient or changing your personal information (name, address etc) you will be asked for proof of identity in the form of your passport, birth or marriage certificate or some other means of identification.
All patients registered with the practice will have a named GP who will have overall responsibility for the care and support that our surgery provides to them. This does not prevent you from seeing any GP in the practice.
When applying for registration, you may express a preference for a particular doctor by entering that doctor's name onto the registration form. The practice will endeavour to comply with your request, but occasionally this may not be possible and in that event the practice will offer you an explanation.
Medical treatment is available from the date of registration. Please contact reception for further information.
